- The video card is probably spoiled or damaged, you have to replace it.
- Windows is DEFINITELY corrupted as a side-effect too, you would need to reinstall again. You may not want to replace the video card immediately, but you have to reinstall Windows, either to another partition or to existing partition.
- Repairing Windows could work, but may inherit problems.
